Overview

Landmark Senior Living Communities, an owner/operator of Assisted Living properties, is seeking a Social Worker to oversee all aspects of resident care in its 75 unit Memory and Traditional Care building in Boston, MA. The position reports directly to the Executive Director.

 

Social Worker Job Purpose: To provide a safe, home-like environment where a person’s quality of life can be maintained through a supportive environment that fosters and encourages their independence through the promotion of good health, an active lifestyle and personalized care.

 

Responsibilities

Social Worker Job Duties:

  • Works as liaison between staff and administration, staff and families, staff and service providers to promote optimal care for the resident
  • Generates and maintains all necessary records/files/reports relating to Medicaid providers such as Group Adult Foster Care, PACE, SCO etc.
  • Assist the RN in conducting initial assessment of all referrals who will require Medicaid assistance
  • Assist the RN in obtaining all documentation necessary for admission
  • Assist the RN in developing Individualized Service Plans
  • Arrange for services that are part of the care plan including transportation to physician’s office and referrals to ancillary care providers
  • Drive process of admitting resident onto appropriate Medicaid programs
  • Intervene in crisis situations and help return resident to baseline
  • Provide support and counseling to resident as needed
  • Assist in all required Department documentation including application and supporting documents, resident records, assessments, written agreements, etc.

Qualifications

Social Worker Qualifications

  • Must possess a minimum of a masters degree in social work and previous experience in working with disabled/elderly adults in a community setting. An equivalent degree and/or experience may be substituted for the educational requirement. Minimum 2 years experience working as a Social Worker in a Long Term Care setting
  • Ability to maintain confidentiality required
  • Must display team-first attitude
